IMPORTANT INFORMATION FOR STUDENTS ENTERING 7th GRADE!
There are 2 state requirements to be completed prior to entering 7th grade per the Nebraska
state law. Please note that if these two requirements are not met, the student WILL NOT be
able to participate in the schedule pick up process until the following requirements are
completed and the paperwork has been turned in to the health office.
1. The Nebraska state law requires ALL 7th grade students to have a physical exam or a
physical exam waiver signed by a parent/guardian PRIOR TO beginning the 7th grade.
Students are REQUIRED to have this paperwork completed and turned into the school
health office. If this paperwork has not been turned we, we will not be able to allow the
student to participate in the schedule pick up process in early August.
Please note that if your child is playing sports, that the sports physical
must be completed AFTER the date of May 1, 2026. The NSAA athletic bylaws require
the physical to be completed after May 1, 2026. If the physical is dated earlier than
May 1, 2026, you will need to obtain a new physical. If you have any questions
concerning this please contact the nurse’s office.
2. IMMUNIZATIONS– 7th graders are required to have a TDAP booster. Please bring the
current immunization records with you to schedule pickup. If the student has not received
the TDAP booster, the student will not be able to get a schedule until the state requirements
have been met.
WE MUST HAVE THE ABOVE ITEMS COMPLETED IN ORDER FOR
THE STUDENT TO RECEIVE THEIR SCHEDULE.

¡INFORMACIÓN MUY IMPORTANTE PARA LOS ESTUDIANTES QUE ENTRAN AL GRADO 7!
Hay 2 requisitos estatales que deben completarse antes de ingresar al grado 7 según la ley estatal de Nebraska. Tenga en cuenta que si no se cumplen estos dos requisitos, el estudiante NO podrá participar en el proceso de recogida de horarios hasta que se completen los siguientes requisitos y se haya entregado la documentación a la oficina de salud.
1. La ley del estado de Nebraska requiere que TODOS los estudiantes de grado 7 se sometan a un examen físico o que un padre/guardián firme la renuncia a un examen físico ANTES DE comenzar el grado 7. Se REQUIERE que los estudiantes completen este papeleo y lo entreguen a la oficina de salud de la escuela. Si esta documentación no se ha entregado, no podemos permitir que el estudiante participe en el proceso de recogida de horarios a principios de agosto.
Padres/Guardianes, tengan en cuenta que si su hijo está practicando deportes, el examen físico deportivo debe completarse DESPUÉS del 1 de mayo de 2026. Los estatutos atléticos de la NSAA requieren que el examen físico se complete después del 1 de mayo de 2026. Si el examen físico es con fecha antes del 1 de mayo de 2026, deberá obtener un nuevo examen físico para poder participar en deportes.
2. VACUNAS– Se requiere que los estudiantes de 7º grado tengan un refuerzo de TDAP. Traiga los registros de vacunas actuales cuando vaya a recoger el horario. Si el estudiante no ha recibido el refuerzo TDAP, el estudiante no podrá obtener un horario hasta que se hayan cumplido los requisitos estatales.
DEBEMOS TENER LOS ARTÍCULOS ANTERIORES COMPLETOS PARA QUE EL ESTUDIANTE RECIBA SU HORARIO O PARTICIPE EN EL PROCESO DE RECOGIDA DEL HORARIO.
