VERY IMPORTANT INFORMATION FOR STUDENTS ENTERING 7th GRADE!!
There are two state requirements to be completed prior to entering 7th grade per the Nebraska state laws. Please note that if these two requirements are not met, the students WILL NOT be able to participate in the schedule pick up process until the following requirements are completed and the paperwork has been turned into the health office.
1. The Nebraska state law requires ALL 7th grade students to have a physical exam or physical exam waiver signed by a parent/guardian PRIOR TO beginning 7th grade. Students are REQUIRED to have this paperwork completed and turned into the school health office. If this paperwork has not been turned in, we will not be able to allow the student to participate in the schedule pick-up process in early August.
Please note that if your child is playing sports, that sports physical must be completed AFTER the date of May 1, 2024. The NSAA athletic bylaws require the physical to be completed after May 1, 2024. If the physical is dated earlier than May 1, 2024, you will need to obtain a new physical. If you have any questions concerning this please contact the nurse's office.
2. IMMUNIZATIONS-7th graders are required to have a TDAP booster. Please bring the current immunization records with you to schedule pick up. If the student has not received the TDAP booster, the student will not be able to get a schedule until the state requirements have been met.
WE MUST HAVE THE ABOVE ITEMS COMPLETED IN ORDER FOR THE STUDENT TO RECEIVE THEIR SCHEDULE!

! INFORMACION MUY IMPORTANTE PARA LOS ESTUDIANTES QUE ENTRAN AL DRADO 7!
Hay 2 requisitos estatales que deben completarse antes de ingresar al grado 7 segun la ley estratal de Nebraska. Tenga en cuenta que si no se cumplen estos dos requisitos, el estudiante NO podra participar en el prceso de recogida de horarios hasta que se completen los siguientes requisitos y se haya entregado la documentacion a la oficina de salud.
1. La ley del estado de Nebraska requiere que TODOS los estudiantes de grado 7 se sometan a un examen fisico o que un padre/guardian firme la renuncia a un examen fisico ANTES DE comenzar el grado 7. Se REQUIERE que los estudiantes completen este papeleo y lo entreguien a la oficina de salud de la escuela. Si esta documentacion no se ha entregado, no podemos permitir que el estudiante participe en el proceso de recogida de Horarios a principios de agusto.
Padres/Guardianes, tengan en cuenta que si su hijo esta practicando deportes, el examen fisico deportivo debe completarse DESPUES del 1 de mayo de 2024. Los estatutos atleticos de la NSAA requieren que el examen fisico se complete despues del 1 de mayo de 2024. Si el examen fisico es con fecha antes del 1 de mayo de 2024, debera obtener un nuevo examen fisico para poder participar en deportes.
2. VACUNAS-Se requiere que los estudiantes de 7 grado tengan un refuerzo de TDAP. Traiga los registros de vacunas actuales cuando vaya a recogerel horario. Si el estudiante no ha recibido el refuerzo TDAP, el estudiante no podra obtener un horario hasta que se hayan cumplido los requisitos estatales.
DEBEMOS TENER LOS ARTICULOS ANTERIORES COMPLETOS PARA QUE EL ESTUDIANTE RECIBA SU HORARIO O PARTICIPE EN EL PROCESO DE RECOGIDA DEL HORARIO.
